Personal Amendment
In accordance with the provisions of the South African Births and Deaths Registration Act, 1992, a South African citizen may apply to amend his/her personal information in the Population Register by virtue of a formal application for personal amendment.
Application to assume another surname
- Completed form BI-529E (www.home-affairs.gov.za/forms/citizenship/bi-529.pdf)
- Completed form BI-196 (majors) (www.home-affairs.gov.za/documents/bi-196.pdf) or BI-193 (minors) (www.home-affairs.gov.za/documents/bi-193.pdf) - Application to Assume Another Surname
- Form BI 24 - Notice of Birth, to indicate the surname to be assumed *
- Copy of Birth Certificate
- Certified copy of Statutory Declaration, Adoption Papers and Court Order or in the absence of these an affidavit motivating the necessity of the change of surname (if applicable)
- Prescribed fee of 6.00 (Minor) to be deposited into the Embassy bank account
- Prescribed fee of 26.00 (Major) to be deposited into the Embassy bank account
Application to assume another Forename
- Completed form BI-529E (www.home-affairs.gov.za/forms/citizenship/bi-529.pdf)
- Completed form BI-85 (www.home-affairs.gov.za/documents/bi-85.pdf)
- Completed form BI- 24 - Notice of Birth, to indicate the new forename *
- Copy of Birth Certificate
- Copy of Statutory Declaration, Court Order or in the absence of these an affidavit motivating the necessity of the change of forename (if applicable)
- Prescribed fee of 6.00 (Minor) to be deposited into the Embassy bank account
- Prescribed fee of 11.00 (Major) to be deposited into the Embassy bank account
